If you’re a new patient, arrive 15 minutes before your appointment to complete new patient paperwork. We will provide those forms to you upon check in.Please be sure to bring photo identification, your auto and/or health insurance information, and your attorney’s information if applicable.

Will my insurance rates go up?

If you’ve been in a car accident in Utah, Personal injury protection (PIP) coverage is required for all drivers in the state. This coverage provides a minimum of $3,000 for medical expenses caused by a car accident. Visiting a doctor after a car accident will not cause your insurance premiums to increase.

Determining who was at-fault in an accident is one of the leading factors that will determine if your insurance rates increase. There are several states with higher-than-average rates following an accident claim and unfortunately, Utah is one of them. Utah drivers who are at-fault in an accident almost always see an increase of about 30% in their auto insurance rates.
